It's that time of year again.....KINDERGARTEN SCREENINGS!!!!!
Here is the information you will need for the screenings.
Held at Midwest Central Primary School- Manito IL.
Monday April 25, 2022 from 4-7pm
Tuesday April 26, 2022 from 8-3pm.
Children are screened by appointments only.
Call Midwest Central Primary at 309-968-6464.
Children must be 5 years old by Sept. 1st.
Please bring the following information at time of screening:
Certified Birth Certificate from the court house
Photo ID
Proof of Residency
Kindergarten Physical
Immunization Records
Lead Screening
Dental Exam
Vision Exam
If your child is currently enrolled at Midwest Central Preschool or attends Mason County Head Start at Midwest Central, they do not need to make an appointment.
Those students will be screened during their regular school day. Those students will still need to have Kindergarten physical, immunization records lead screen, dental and vision exam.
