Certified Birth Certificate

Photo ID

Proof of Residency

Before starting Kindergarten, your student will need:

Physical/ Immunization Records/ Lead Screening

Dental Exam

Vision Exam

Students currently enrolled at Midwest Central Preschool or Mason County Head Start at Midwest Central do not need an appointment. These students will be screened during the day.
